Founder

Dr. Annette Smith

PT, DPT · Owner & Lead Clinician


Annette founded Mountain Laurel Physical Therapy after years in Iowa outpatient clinics where she watched patients get passed around from one technician to the next. She wanted a different kind of practice — one where every patient works directly with a doctor of physical therapy, every visit. That promise still defines the clinic today.

Her clinical focus sits at the intersection of vestibular therapy, concussion rehab, and orthopedic care. She trained with infrared video-frenzel goggles to pinpoint vestibular dysfunction and pairs that diagnostic rigor with old-fashioned hands-on therapy.

Dr. Katherine Greening

PT, DPT


Katherine believes the best plan is the one a patient actually understands. She's direct, warm, and allergic to medical jargon — the kind of clinician who'll draw on a whiteboard to show you exactly what your rotator cuff is doing and why it hurts.

Her caseload ranges across orthopedics with a special soft-spot for female athletes, runners coming back from injury, and complex chronic-pain presentations that have bounced between providers without a clear answer.

Dr. Chris Wooley

Chris brings more than a decade of outpatient orthopedic experience to Mountain Laurel. Since earning his DPT from Clarke University in 2013, he has practiced exclusively in outpatient orthopedics, treating everything from common strains to highly complex post-surgical cases.

He completed an 18-month certification to become a Certified Manual Physical Therapist and is Level II certified in functional dry needling, with more than 1,000 needling sessions performed. His approach centers on helping patients understand their condition, identify the root cause, and move forward with a clear, individualized plan.

Dr. Sadie Teno

PT, DPT · Vestibular & Pelvic Floor Specialist


Sadie's practice sits at the crossroads of two populations most clinics underserve: patients with vestibular dysfunction and women navigating pelvic-floor concerns. Her care style is calm, curious, and methodical — a rehab philosophy modeled on a good hiking guide: know the terrain, check the weather, pack what you need, and celebrate the milestones along the way.

On the vestibular side she treats BPPV, vestibular hypofunction, and persistent post-concussion symptoms. On the pelvic-floor side she supports postpartum recovery, pelvic pain, urinary urgency and leakage, and the "I just haven't felt like myself since the baby" cases that take patience, creativity, and a genuine therapeutic relationship.
